Uptrack app for iPhone and iPad

Uptrack has been designed to support patients with PAH who are starting treatment with Uptravi.

This app is a digital version of the Titration Guide to support you during the titration phase. It is intended to be a companion tool for you, helping you remember to take Uptravi during the titration phase.

Uptrack includes many features and resources to support you and your healthcare team to manage and keep track of your disease and treatment during titration. By using the Notes, Appointments and Goals sections, you can keep all the information related to your treatment in one place, for easy reference.

The app is not intended in diagnostics or treatment decisions. Uptrack is intended as a support tool and does not replace the opinion and interaction with your healthcare team.

An app code can be supplied to you by your Healthcare Team.
